Dear Parent: 

The Maumee Police Division, in conjunction with the Maumee Board of Education, would like to invite your preschool age child to participate in our Safety City Program. The sessions will be held on June 14 - 25th and July 12 - 23rd.  Morning classes are from 8:00 a.m. – 10:30 a.m. and afternoon classes are from 11:30 a.m. – 2:00 p.m., Monday through Friday. 

Safety City is located at Fort Miami Elementary School, 716 Askin Street, in Maumee.  Transportation to and from Fort Miami School will be the responsibility of the parent.

Any Maumee child who will be entering kindergarten in the fall of this year is welcome to attend this valuable program.  The cost is $10.00 for residents who live within the city limits of Maumee (boundaries are I-80/90, I-475/Rt. 23, & the Maumee River, as well as the recently annexed area west of I-475) or children attending schools located in Maumee.  The fee for non-residents is $15.00.  Registration and fingerprint release forms are available at the Maumee Police Division, 1283 Conant Street, or can be printed below.  All fingerprints taken are given to parents at the end of the course.

The Safety City Program is a two week course consisting of 2.5 hour daily sessions Monday through Friday.  Each session is divided into three parts.  One period consists of classroom instruction by a teacher, in which the children learn songs, games, and crafts related to the daily lessons.

During the second period, the children meet with community resource people such as a fireman or school bus driver, who explain their functions, equipment, and how a child can best assist them.

Third period is spent outside in “Safety City” with police officers.  The children play the parts of pedestrians and vehicular traffic, learning the importance of obeying traffic signs and signals.

Positive reinforcement by parents can develop these lessons into good safety habits.  We look forward to seeing your child at Safety City.    If you have any questions, please call Officer Jeff Medere at 419-897-7028.

CONSENT, RELEASE AND WAIVER

The undersigned parents and/or guardians of ______________________________________, a minor, for and in consideration of the services provided by the Police Division of the City of Maumee, Ohio, and for other good and valuable consideration, the receipt of which is hereby acknowledged by the undersigned, do hereby consent to have the Maumee Police Division fingerprint said child and do hereby waive, release, and forever discharge the City of Maumee, Ohio, a municipal corporation, the Maumee Police Division, and their agents, successors, and assigns from any and all claims and causes of action of any kind or nature which the undersigned may have, or in the future, can, shall, or may have on account of any and all damages, losses, or injuries to persons or property, or both, known and unknown, resulting or to result from fingerprinting of said child.

It is hereby acknowledged that the finger printing of said child is being done at the voluntary request of said child and the undersigned.  All fingerprint cards will be given to the undersigned.

The undersigned hereby declare that the terms of this consent, release, and waiver have been completely read and are fully understood and voluntarily accepted for the purpose of aforesaid and for the express purpose of precluding forever any further or additional claims arising out of aforesaid fingerprinting.
