2005 Holiday Hustle 5K – Sponsorship Information 

All profits from the 2005 Holiday Hustle 5K race will benefit the Maumee Senior Center whose mission is “to create an atmosphere which affirms the dignity and self-worth of the older population by serving their physical, emotional, and economic well-being while encouraging their independence and continued participation in the community.”  The center is partially funded by the Lucas County Senior Services Levy administered through the Area Office on Aging of Northwest Ohio.  To learn more about the Maumee Senior Center, please visit their website at www.maumeeseniorcenter.com. 

Sponsorship Levels:

$250 – One sponsor only with company name included on the Holiday Hustle 5K race bib.  The first company responding will be awarded this distinction. 

$150 – Company name / logo included on the back of the Holiday Hustle 5K race t-shirt distributed to race participants. 

Sponsorship may be in the form of a monetary and / or in-kind donation and must be received by Friday, September 20, 2005 to be included on the t-shirt.  A $250 and all in-kind contributions must be coordinated through the City of Maumee by contacting Mr. John Jezak, Administrator, by email at jezak-john@maumee.org or by phone at 419-897-7100.  

Your company name will be printed in a font style and size chosen by the City of Maumee. If your company has its name and / or logo available as camera-ready art or in a digitized format, please check mark the proper box below.  You will be contacted with further instructions prior to Friday, October 14, 2005.
